Your first appointment includes a full evaluation of your condition. Your physical therapist will:

Review your treatment options
If time allows, treatment often starts the same day. To help us give you the best care, please arrive on time for your appointment.

Get a Referral/Prescription

Take the Referral form to your physician
Have your physician fill out the form and FAX the referral directly to our office

Schedule Appointment
Once we receive the referral from your physician our office will call you to schedule

Once you are schedule you will receive your digital patient intake forms through your email. Please fill these forms out prior to your appointment.
